Niagen IV vs NAD+ IV: What's the Difference, and Which One Fits You?
If you have been reading about longevity protocols, you have probably run into both of these names. NAD+ IV therapy has been the headline treatment for years, and Niagen IV is the newer arrival that keeps coming up in the research world. They are closely related, but they are not the same thing, and the differences matter when you are deciding where to start.

What Niagen IV actually is
Niagen is a patented form of nicotinamide riboside, or NR. It is not NAD+ itself. It is a precursor involved in NAD+ metabolism, so it follows a different biological path from infusing NAD+ directly.
Oral NR has published human research, while research on commercial IV NR protocols is still early. A small 2026 retrospective pilot directly compared IV NR and IV NAD+, but it was not a large randomized trial and cannot establish long-term effectiveness.
What NAD+ IV is
An NAD+ drip delivers the finished coenzyme directly into your bloodstream. NAD+ is a co-factor every cell in your body uses to produce energy and repair DNA, and levels decline measurably with age.
Evidence for commercial IV NAD+ protocols is also limited. The practical difference guests notice is the infusion experience: NAD+ drips run slowly because faster rates can feel uncomfortable, and a full session can take anywhere from ninety minutes to four hours depending on the dose.
The practical differences
Comfort. This is the big practical difference. In a small retrospective pilot, the IV NR group reported milder infusion symptoms and shorter infusion times than the IV NAD+ group. That is preliminary evidence, not a guarantee that every person will have the same experience.
Time. A Niagen session at Health Hax runs about 90 minutes, and we follow it with a cleansing IV infusion to replenish. NAD+ can take up to four hours at higher doses.

- Start with NAD+ if you want the direct protocol, you are comfortable with a longer and more intense session, or you want to begin with a lower-cost IM shot and work up.
